Our Mobile Medical Intervention Team in Tulsa and Street Medicine Team in Oklahoma City provide high-quality, low-barrier, non-judgmental, patient-centered care by taking care to people where they live.

What We Do:

Our PA and APRN improve access to high-quality, low-barrier primary care for acute and chronic conditions at no cost to our patients.
Our nurses improve adherence to prescription medications and follow-up care through care coordination.
Our case managers improve access to community services by collaborating with community partner agencies and advocating for our patients through service navigation.
Our friendly, non-judgmental Street Medicine Outreach and Engagement (SMOE) activities provide warm smiles and welcoming conversation inviting people to receive services where they live.
Our teams address the financial, cognitive and structural barriers that make it harder for people experiencing homelessness to get health care early, prevent illness, and receive the care they need.
