Our Mobile Medical Intervention Team in Tulsa and Street Medicine Team in Oklahoma City provide high-quality, low-barrier, non-judgmental, patient-centered care by taking care to people where they live.

What We Do:

  • Our PA and APRN improve access to high-quality, low-barrier primary care for acute and chronic conditions at no cost to our patients. 
  • Our nurses improve adherence to prescription medications and follow-up care through care coordination. 
  • Our case managers improve access to community services by collaborating with community partner agencies and advocating for our patients through service navigation. 
  • Our friendly, non-judgmental Street Medicine Outreach and Engagement (SMOE) activities provide warm smiles and welcoming conversation inviting people to receive services where they live. 
  • Our teams address the financial, cognitive and structural barriers that make it harder for people experiencing homelessness to get health care early, prevent illness, and receive the care they need.
